Your agreement with vaginalestrogencreamonline.com when you use this website, submit an online intake, or receive a prescription through our service.
1. Accepting these Terms
These Terms & Conditions form a binding agreement between you and Midwest Mind & Body Healthcare LLC ("we", "us", "the practice"), which operates vaginalestrogencreamonline.com (the "Site"). By using the Site or submitting an intake, you agree to these Terms and to our Privacy Policy and Notice of Privacy Practices.
If you do not agree, do not use the Site or submit an intake.
2. Not medical advice, not an emergency service
If you are experiencing a medical emergency, call 911. Do not use this Site to seek emergency care. If you are in mental health crisis, call or text 988 (Suicide & Crisis Lifeline).
The content on this Site is general health information, not medical advice, and it is not a substitute for evaluation by a clinician who knows your history. Reading this Site does not create a patient-provider relationship. That relationship begins only when a licensed clinician accepts and reviews your intake.
3. Who may use this service
To use this service you must:
- Be 18 years of age or older.
- Be physically located in one of the states where our clinician is licensed at the time of your intake: Nebraska, Iowa, Colorado, Illinois, Kansas, Arizona, Utah, Idaho, Montana, South Dakota, North Dakota, Kentucky, Vermont, New Hampshire, Maine, or New Mexico.
- Be submitting an intake for yourself, not on behalf of another person.
- Be able to provide truthful, accurate, and complete health information.
If your state of residence changes, tell us before submitting an intake or a renewal. Submitting an intake from a state where we are not licensed does not create a patient relationship, and no prescription will be issued.
4. How asynchronous telehealth works
This is an asynchronous (store-and-forward) telehealth service. You complete an online intake; a licensed clinician reviews it later, typically within 1–2 business days. There is no live appointment and no video visit.
- Benefits. Convenience, privacy, and access for patients who do not need or prefer a live visit for this specific condition.
- Limitations. Asynchronous review has inherent limits: the clinician does not physically examine you and relies entirely on the accuracy of what you report. Some conditions cannot be appropriately evaluated or treated this way. In those cases the clinician will decline to prescribe and, where appropriate, will suggest an alternative.
- Privacy. Intake is collected on a secure, HIPAA-compliant form hosted by Hushmail under a Business Associate Agreement. No health information is collected on this website. See our Privacy Policy.
5. Prescribing standards
Treatment is not guaranteed. A prescription is issued only when a licensed clinician determines it is clinically appropriate for you. This is a clinical judgment and is not subject to negotiation, appeal, or refund dispute.
We prescribe commercially available, FDA-approved vaginal estradiol only. We do not prescribe compounded vaginal estrogen through this service.
We will not prescribe through this service if you have:
- A current or recent history of breast cancer or other estrogen-sensitive cancer;
- Active thromboembolic disease;
- Undiagnosed vaginal bleeding, without OB-GYN clearance first;
- Known hypersensitivity to the medication; or
- Any other condition the reviewing clinician determines makes vaginal estrogen unsafe for you.
Vaginal estrogen therapy is generally not used during pregnancy or breastfeeding. Tell us if you are pregnant, could be pregnant, trying to conceive, or breastfeeding.
We do not prescribe controlled substances through this service.
6. Fees, billing, and refunds
6.1 The fee
The clinical fee is a one-time $50 charge covering the clinical review and, if approved, a prescription that is good for a full year. There is no subscription, no membership, no auto-renewal, and no card retained for recurring billing.
6.2 When you are charged
You are not charged until your prescription is approved. Submitting an intake costs you nothing. If a clinician determines that treatment is not appropriate for you, there is no charge.
Payment is processed by Stripe. We do not store your full card number.
6.3 Refunds
Because you are not charged unless a prescription is approved and issued, the fee is generally non-refundable once the prescription has been transmitted to your pharmacy — at that point the service you paid for has been delivered. Dissatisfaction with a clinical decision, or a change of mind after a prescription is issued, is not a basis for refund.
If you believe you were charged in error, contact us within 30 days at 531-217-5257 and we will review it.
6.4 Renewal
Nothing renews automatically. If you want to continue after your prescription year, you complete a brief renewal intake and pay the same $50 fee for another year. The fee may change; the price displayed on the pricing page at the time you submit controls for that intake.
6.5 Insurance
The $50 clinical fee is a self-pay charge and is not billed to insurance. Your insurance, if any, applies to the medication at your pharmacy. We make no representation about what your plan will cover; pricing figures on this Site are estimates, not quotes.
7. Pharmacy and medication
You choose your pharmacy. If a prescription is issued, we transmit it electronically to the pharmacy you designate. You pay that pharmacy directly for the medication.
We have no financial relationship with any pharmacy or drug manufacturer. We do not receive payment, commission, rebate, or any other consideration based on where you fill your prescription or what you pay. Pharmacies are treatment-related disclosures, not our business partners.
Once medication is dispensed, the pharmacy is responsible for its handling, labeling, counseling, and storage. Questions about the physical product should go to the dispensing pharmacy.
8. Your responsibilities
- You must provide truthful, accurate, and complete information on the intake and in any follow-up. Clinical decisions depend entirely on what you report; withholding or misrepresenting information may cause harm and may result in refusal or termination of services without refund.
- Use the medication only as prescribed, and only for yourself. Do not share it.
- Tell us promptly about new symptoms, new diagnoses, new medications, pregnancy, or any adverse reaction.
- Maintain a relationship with a primary care provider or OB-GYN for care outside the scope of this service.
- Keep your contact information current so we can reach you about your care.
9. Communications and SMS
By providing your email address and phone number on the intake, you agree that we may contact you about your care, your prescription, and your account.
Express written consent is required first for text messages. Submitting the online intake does not opt you in to SMS. You will be given a separate opportunity to consent to receive SMS or automated voice messages. You will receive care-related text messages only after you opt in, and you may opt out at any time by replying STOP. Message and data rates may apply.
Do not send protected health information by ordinary email. Email is not a secure channel.
10. Website and intellectual property
All content on this Site — text, layout, design, and code — is owned by Midwest Mind & Body Healthcare LLC and protected by copyright. You may view and print pages for your personal, non-commercial use. You may not copy, republish, scrape, or use this content commercially without written permission.
You agree not to interfere with the Site, attempt to gain unauthorized access, or use automated means to extract content.
11. Disclaimers and limitation of liability
The Site is provided "as is" and "as available" without warranties of any kind, express or implied, including any implied warranties of merchantability, fitness for a particular purpose, or non-infringement. We do not warrant that the Site will be uninterrupted or error-free.
Nothing in this section limits our liability for professional negligence in the delivery of clinical care, which is governed by the law of the state where you received care and by our professional liability coverage. Nothing in these Terms waives any right you have under applicable state or federal law, and no provision applies to the extent the law prohibits it.
To the fullest extent permitted by law, and except for claims arising from clinical care, our total aggregate liability arising out of your use of the Site will not exceed the amount you paid us in the twelve months preceding the claim.
12. Governing law and disputes
These Terms are governed by the laws of the State of Nebraska, without regard to conflict-of-law principles. Any dispute not resolved informally shall be brought in the state or federal courts located in Sarpy County, Nebraska, and you consent to that jurisdiction.
This provision does not apply to any claim you may bring before a state licensing board, the U.S. Department of Health and Human Services Office for Civil Rights, or any other regulator. Those rights are unaffected.

13. Changes and contact
We may update these Terms from time to time. The "Last Updated" date at the top shows when the current version took effect. Material changes will be posted on this page for a reasonable period. If you continue to use the Site after a change, you agree to the updated Terms.
