Notice of Nondiscrimination & Language Assistance
Required by Section 1557 of the Affordable Care Act. Free language assistance and free auxiliary aids are available to you at no cost.
1. Our commitment
Midwest Mind & Body Healthcare LLC, which operates vaginalestrogencreamonline.com, complies with applicable federal civil-rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), age, disability, or sex — including pregnancy, sexual orientation, gender identity, and sex characteristics — consistent with Section 1557 of the Affordable Care Act, 42 U.S.C. § 18116, and its implementing regulations at 45 C.F.R. Part 92.
We do not:
- Exclude anyone from participation in our services, or deny the benefits of our services, based on any of the protected characteristics above.
- Treat anyone differently from others based on those characteristics.
- Deny reasonable modifications to policies, practices, or procedures when needed to provide equal access to our services.
A note specific to this service. Local vaginal estrogen is prescribed for genitourinary symptoms. Eligibility for this service is determined by clinical appropriateness and state licensure alone. It is not conditioned on your gender identity, sexual orientation, marital status, or the reason you are seeking treatment.
2. Services at no cost
Auxiliary aids and services for people with disabilities
We provide appropriate auxiliary aids and services, free of charge, to help you communicate effectively with us. These may include:
- Qualified sign-language interpreters, in person or by video remote interpreting, for scheduled visits.
- Real-time captioning for video visits.
- Written materials in large print, Braille, audio, accessible electronic formats, or other alternative formats.
- Extended time when needed for effective communication.
- Assistance with our website and with completing your intake, including completing it with you by phone. See our Accessibility Statement.
Language assistance for people with limited English proficiency
We provide language-assistance services, free of charge, so you can communicate with us in the language you understand best. These may include:
- Qualified medical interpreters, in person, by phone, or by video, in your preferred language.
- Translated written materials on request for key documents.
- Bilingual staff or bilingual telehealth options where available.
We do not ask family members or friends to interpret. You are entitled to a qualified interpreter at no cost, and we will arrange one. Minor children are never used as interpreters.
3. How to request them
Call 531-217-5257 and tell us what you need. Advance notice helps us arrange interpretation, but we will do our best to accommodate last-minute requests.
You may also email Info@midwestmindandbodyhealthcare.com. Please do not include health information in email.
There is never a charge for these services, and requesting one will not delay or disadvantage your care.
4. Filing a grievance with us
If you believe that Midwest Mind & Body Healthcare has failed to provide any of the services above, or has discriminated against you on the basis of race, color, national origin, age, disability, or sex, you may file a grievance with us.

You may submit a grievance in person, by mail, by phone, or by email. If you need help filing, the Section 1557 Coordinator is available to assist you. We will investigate, respond in writing, and give you our decision and the reasons for it.
We will not retaliate against you for filing a grievance.
If your grievance concerns the Section 1557 Coordinator directly. You may send it to the same mailing address marked "Attention: Owner — Confidential," or file directly with the U.S. Department of Health and Human Services as described in Section 5. You are never required to raise a complaint with the person it concerns.
5. Filing with HHS Office for Civil Rights
You may file a civil-rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR). You are not required to file with us first.

6. Language assistance taglines
The following taglines let non-English speakers know that free language assistance is available. For any listed language, please call 531-217-5257 and we will arrange an interpreter at no cost to you.
If your language is not listed, call 531-217-5257 anyway. We use a telephone interpreter service that covers far more languages than we can print here.
7. Availability of this Notice
This Notice and the taglines above are posted on this website and are provided in hard copy on request.
